Credentials

You may qualify for licensure by Credentials

Licensure by Credentials is the pathway to licensure for an applicant who holds a license in another state, who has passed all parts of the National Board Dental Hygiene examination given by the Joint Commission on National Dental Examinations, and has successfully completed/passed a clinical competency exam acceptable by the Board as stated in guidance document 60-26.

Every candidate shall:

• Have graduated from or have been issued a certificate by a CODA or CDAC accredited program of dental hygiene;
• Be currently licensed to practice dental hygiene in another jurisdiction of the United States and have clinical, ethical, and active practice for 24 of the past 48 months immediately preceding application for licensure;
  (Must provide written verification that you have had “clinical, ethical and legal practice for 24 months out of the previous 48 months immediately preceding application for licensure”. A notarized statement from each dentist and/or agency that has employed you within the four years immediately preceding the date of your application. The statement must include the printed name and address of the employer, must include the information noted on the Board’s employment verification form, and must state the months, days and years of your employment. Only original, notarized statements are accepted.)
• Be certified to be in good standing from each state in which he is currently licensed or has ever held a license; and
• Not have committed any act that would constitute a violation of § 54.1-2706 of the Code.

Reinstatement Application

When a license has not been renewed within one year of the expiration date, or a license has been suspended or revoked, a person may apply to reinstate their license by submission of a reinstatement application.

Reactivate a Dentist License

If you hold a Current Inactive Dentist license and wish to return to practice. Please complete the Reactivation application for Dental Hygienists.

Applications for reactivation should be mailed to:

Reapply

Applications will remain on file with the board for one year from the date of receipt. If, at the end of one (1) year, a license is not issued, the applicant shall reapply in accordance with the requirements of the Regulations.
